If paying by Cheque, you must print and complete the application form then mail or deliver the form and cheque to City Hall Woodstock (500 Dundas St. Woodstock, ON N4S 0A7 P.O. Box, 1539 | Attn: Kristen Brodhagen).

Cheques can be made payable to the City of Woodstock.

ATTENTION – No more than two of the same vendors will be accepted.
