Please complete this form if you are applying for a Liquor Permit for:

New Liquor Sales Licence
or adding outdoor areas to an existing Liquor Sales Licence

The Clerks Department gather the comments from the Building, Police and Fire Departments on your behalf. We will notify the health department of your application (you may need to obtain comments from them directly, this is dependent on their discretion).

Once all department comments have been collected, you will be notified via the e-mail address you have provided on the form.

After you receive your information, follow the instructions provided by the Alcohol and Gaming Commission of Ontario.

There is a $50.00 non-refundable fee for completion of the Municipal Information Form.
